Whistleblowing report form – Certificato IWZ

Please complete this form to submit your report to Certificato IWZ. The report will be handled by the Supervisory Body (OdV), which, once verified the relevance of the report for the purposes of Legislative Decree 231/01 and applicable regulations, will carry out the appropriate analysis ensuring the confidentiality of the whistleblower's identity, where provided.

By submitting the report, the whistleblower undertakes to provide truthful and, where possible, documented information, refraining from carrying out autonomous investigative activities.

Any personal data provided will be processed by Certificato IWZ in compliance with the applicable data protection legislation and the specific privacy notice relating to the whistleblowing channel. Providing identifying data is optional; in the case of an anonymous report, it will not be possible to contact the whistleblower for any further clarification.
